This link will allow you to fill in your intake forms electronically as well as e-mail our office securely and set up appointments after your initial appointment. ..Once you have called my office and set up an appointment, my office will e-mail you an id and password to get the intake paperwork through this secure portal. Once you complete all the paperwork, it gets sent back to us and we will have it before your appointment. You can also e-mail us through the portal as well as add/change appointments.
